Hospital at Home Vascular Access Cases: A Hospital at Home patient had initially presented at a hospital Emergency Department and was diagnosed with several comorbidities, including sepsis and thrombocytopenia, a condition that occurs when the platelet count in a patient’s blood is too low. After reviewing the patient’s situation and treatment plan, the care team determined she was a candidate for their Hospital at Home program, and the patient was able to return home to continue her treatment plan. After several days of in-home treatment, labs were ordered to confirm the patient’s healing progress. The medical provider arrived at the patient’s home to draw blood, but unfortunately, the patient’s veins were depleted, which made gaining vascular access particularly challenging. After more than 10 unsuccessful attempts, which resulted in numerous and unnecessary painful needle sticks, Vascular Wellness was consulted to assist. After assessing the patient’s vasculature, the clinician then quickly and effortlessly achieved vascular access on the first attempt utilizing ultrasound guidance. The patient was thrilled and stated, “I didn’t even feel it, I was expecting to be poked a lot more that that!” She also told the Vascular Wellness nurse clinician that she had made her day.

Vascular Wellness clinicians are Vascular Access Board Certified, skill tested, and verified semiannually, and can rely on the breadth of their intensive training to achieve successful access every time, including in difficult intravenous access (DIVA) patients or those with fragile veins, which is common in the elderly population or patients with comorbidities.
Our clinicians are salaried rather than a pay-per-procedure model, and are encouraged to spend all time necessary to perform a comprehensive medical history review to ensure the clinically appropriate line before sticking, to answer all questions about the procedure and care and maintenance, and to ensure the patient is comfortable and has the best possible experience.
